This page will relay information about Mephisto, the Act 3 Boss of Diablo 2, especially concerning information about farming him for unique and set items.
Mephisto, the Lord of Hatred, is your reward for slogging through the jungles of Act 3. Like most bosses, there’s little incentive to farm him on Normal difficulty, but he’s arguably the most interesting boss to farm for new players on Nightmare and Hell difficulties. There’s one issue with reaching him, however: you’ll have to go from the Durance of Hate Level 2 Waypoint and seek out the Durance of Hate Level 3, where Mephisto dwells, a task complicated considerably by the presence of Undead Stygian Dolls, which plague the Durance of Hate. These critters are notoriously dangerous, as they’ll explode when killed, dealing 10% of their Life as physical damage, and on Hell difficulty where they have thousands of Life, this can grow very painful. This makes the Sorceress the ideal Mephisto-runner, as she can (with high enough Cast Rate - ideally 75% or more) Teleport brazenly through the Durance of Hate, bypassing all the nasty little bombs along the way.

Mephisto has a wide array of attacks, including Lightning Bolt, Frost Nova, Charged Bolt and (rarely) Blizzard. He can also shoot out a frozen ball, which moves rather quickly and deals a lot of damage, and he can conjure poison gas around him. Given his heavy focus on ice and lightning, it’s surprising he’s not more resistant to both, being fairly vulnerable to cold damage until Hell difficulty, where his defenses shape up. He’s also vulnerable to physical damage across all difficulties. As long as you have decent cold and lightning resistance, he shouldn’t be too dangerous - getting to him is the bother, and the vampires and council members he keeps in the vicinity can be a real chore if steps aren’t taken to eliminate or avoid them.
Nightmare Mephisto is one of the first, best farming opportunities in the game, as his Monster Level (mLv) is high enough to allow him to drop an impressive list of mid-tier gear, including Peasant Crown, Skin of the Vipermagi, Skullder’s Ire, War Traveler, Jalal’s Mane, The Oculus, Homonculus, Herald of Zakarum, Vampire Gaze, Titan’s Revenge and Shaftstop among others. Farming Mephisto for mid-tier gear is almost essential for most builds to compete in Hell difficulty, and there’s so many build-defining (or at least build-starting) items on offer here that it makes Nightmare Mephisto a very tantalizing target.
Hell difficulty is more of the same, and while Mephisto can’t drop every item in the game, he can drop items like Arachnid Mesh and Harlequin Crest along with high-end set items. Nightmare Mephisto is much easier to run, but Hell Mephisto is more lucrative, and being able to confidently take him down in a timely manner is a good sign your character is Hell difficulty competent.
The Blizzard Sorceress is arguably the expert Mephisto hunter, as Mephisto’s AI isn’t up to the task of navigating around the moat bisecting his level, allowing the Sorcesses to lure him near the moat and attack him from out of range on the far side of the moat. While useful, the “moat trick” isn’t necessary, and once you’ve got a mercenary who can survive long enough for you to kill Mephisto, it can be abandoned, as there’s less chance you’ll be attacked by other monsters on the level if you just teleport up to Mephisto and engage him on the northern end of the level.
