Diablo 4 - Depths of Despair Walkthrough

As you pursue Lilith through Sanctuary you’ll stumble across side quests, marked by blue exclamation points on the map. Completing these will earn you various rewards, usually including (but not limited to) XP, gold and Renown. This page provides a walkthrough for the side quest "Depths of Despair" iconDepths of Despair, a side quest in the Fractured Peaks region in Diablo 4.

After completing “Malady of Despair”, zone out out !Kyovashad and return to find that Sister Octavia needs your services again.

How to Start Depths of Despair in Diablo 4

The second quest in the "Malady of the Soul" iconMalady of the Soul questline, before you can start Depths of Despair you must complete the aforementioned Malady of the Soul. Once done, travel away from "Kyovashad" iconKyovashad if necessary, which should trigger this quest to spawn - you’ll need to seek out Sister Octavia along the eastern edge of Kyovashad. Talk to her to begin this quest.

How to Exorcize Kvera’s Husband

Another resident of Kyovashad has apparently fallen afoul of demonic possession - Kvera’s husband, Vasek - and once again you’ll need to be Sister Octavia’s wingman during an exorcism. After the chatter subsides, enter a house to find an Ominous Cellar, where Kvera’s husband has been restrained.

Enter the cellar and smite some "Carver" iconCarvers, only to subsequently discover that Kvera’s husband has fled. Leave the cellar and talk to Kvera, then follow a blood trail through the streets to find that Vesak has fled into Kyovashad’s Sewers. Enter the sewers and you’ll find that, like many video game sewers, Kyovashad’s underbelly is a massive, cavernous structure full of nasty critters. Makes one wonder if the city’s demonic possession issue is due to the fact that the sewers are filled with demons… who knows what kinds of vile promises these infernal beings are making from the toilet?

Anywho, your goal is to track down Vasek, who will lead you on a merry chase through the sewers. You’re not going to catch him before he reaches his destination, so focus on killing your way through the sewers until you reach the Wastewater Basin. Driven to this suspiciously arena-shaped chamber, Vasek will be cornered; place Sister Octavia’s chalice and she’ll go about the ritual. There’s no happy ending this time around, and you’ll end up having to fight a demon named "Demotath" iconDemotath.

In our game Demotath - a hulking, winged monstrosity that looks a lot like an oversized "Oppressor" iconOppressor, albeit one armed with a massive axe. He’s more bark than bite, however, being surprisingly fragile for his size. His axe swings can be painful, especially with repeated hits, as he can inflict the Vulnerable debuff. In addition, as an elite enemy he spawned with the Frozen and Chilling Wind traits, the later of which will allow him to manifest frigid torrents periodically, an act that’s well telegraphed and hence easy to avoid. Defeat him and the demon will taunt you for failing to achieve your goal, after which you’ll need to exit the sewers with an injured Sister Octavia. Once outside, talk to your stricken companion to complete the quest, earning a Cache and a variable amount of XP and gold.
