As you pursue Lilith through Sanctuary you’ll stumble across side quests, marked by blue exclamation points on the map. Completing these will earn you various rewards, usually including (but not limited to) XP, gold and Renown. This page provides a walkthrough for Malady of the Soul, a side quest in the Fractured Peaks region in Diablo 4.
How to Start Malady of the Soul in Diablo 4¶
To start this quest, search a side street in Kyovashad between the Healer (northwest) and the Purveyor of Curiosities (southeast) to find Sister Octavia and Petr, a distraught man whose son has apparently been possessed. Talk to them and the quest will begin. Note that Malady of the of the Soul is the first quest in a three part questline.
Help Sister Octavia Exorcise Petr’s Son¶
After talking to Sister Octavia, follow her into the nearby house and enter the Frigid Cellar area and talk to her again. She’ll give you a “holy chalice” and instruct you to place it near a bed in a room to the northeast. Do so and after some more chatter Petr and Sister Octavia will enter the room - talk to the latter again to get the exorcism started.
Sister Octavia will exchange pleasantries with the demon possessing Petr’s son, after which some Carvers will spawn and attack. Dispose of these pests and Sister Octavia will persist with her ritual, driving the possessing demon out, and he’s not happy about being removed from his host. You’ll now have to defeat another demon, Xul’Goth, an elite Carver who possessed the Cold Enchanted and Summoner mods when we encountered him. He hits hard and you don’t have a ton of room to move around, especially given his oversized sword, but he’s not terribly resilient, himself. Stand up to the demon and smash him, after which you’ll get a rare happy ending in a Diablo game.
Following the successful conclusion of the exorcism, Sister Octavia will wryly comment on expected demonstrations of Petr’s renewed faith. Leave the cellar and talk to Sister Octavia one more time and she’ll mention having more work for you in the future. For completing this quest you’ll earn an Herb Cache and a variable amount of XP and gold.
