The Barber Heart in Diablo 4 is one of the more iconic additions of Season 1. This Heart is very powerful, which enables for even more multipliers, on top of your crit damage bucket, allowing crit builds to grow even stronger. If you’re the type of character that wants to use Crit as part of the playstyle, here’s everything you need to know about the Barber Heart in Diablo 4.

The Barber Heart is a Wrathful Malignant Heart, which drops from World Tier 3 onwards. If you’re a crit focused build, then you will want to use this Heart. The reason why it is so important is because of the Hearts power that allow you to ramp even more damage once you crit after several seconds, depending on its roll. This means subsequent crits will get added to the multiplier, and any other form of passive damage you deal. This means the heart is bolstering the damage multiplier in your regular damage bucket, and your crit damage bucket, which is a very strong synergy indeed.
If you’re a build that crits easily, such as a Chain Lighting Sorcerer, or a Bone Spear Necro, this is easily the best Malignant Heart for your build, when you can get your hands on it. These are just two builds with in-built crit modifying, but, there’s likely more builds out there that get great usage out of Crit damage.
Also, we highly recommend pairing The Barber Heart with the Tempting Fate Vicious Heart. This increases Critical Strike Damage by a sizable amount. So, when you crit, that damage is modified by Tempting Fate, and then stored for several seconds. Subsequent crits within the timer created by Barber, and modified by Tempting Fate will then stack, and then get modified by the Barber’s damage multiplier. Essentially you are getting even more bang for your buck if you happen to crit multiple times within the Barber’s stalling window.
This leaves one piece of jewelry spare for you to slot in any other from the Malignant Heart list of powers. That way, you get to add some other build variation, such as The Picana for Chain Lightning, Lightning Storm, or Stormbeast Werewolf builds. Or perhaps something like The Sacrilegious for Necromancers, or anything else you can think of. Perhaps you prefer the Prudent Heart if your build needs more defensiveness. The options are still open for you.
How To Farm For The Barber Heart In Diablo 4¶
The main thing you need to know about Wrathful Hearts in Diablo 4 is that they are notoriously the hardest to farm. Wrathful Hearts do not have a dedicated craft for an Invoker, and they are also the rarest type for Corrupted Elites to spawn as. So, you are going to need to be lucky by crafting the Uncertain Invoker, or by crafting the Uncertain Heart and getting one of the 5 of the Wrathful Hearts usable by your class.
In addition, the Barber, among a few other types of Wrathful Hearts only begin to spawn in World Tier 3, with another in World Tier 4. So, ideally, you will want to farm these from World Tier 3 onwards. It’s great while you are in World Tier 3, as they will scale with your level. However, when you start entering World Tier 4, you need to move up so they scale with your level, but, the drop chance reduces as you get World Tier 4 drops in the pool too. It’s something to be weary of, but its the nature of the game’s progression system.
A decent remedy for this issue is to use your Smoldering Ashes on the Malignant buff. This grants better percentage chances on some of the rarer Malignant Heart drops to appear. The Barber is one of them, as it is classed as a Super World Tier 3 item. And so, you increase your odds of getting one of them to drop as you play the game.
As for specific farms, you can run Malignant Tunnels, and follow the routes to find Wrathful Corrupted Elites. You can also run to the end of the Tunnels and look for the Wrathful outgrowths, so you can consume your Wrathful Invoker on it. That way you guarantee get a Wrathful threat. A decent one to farm is the Fissure of Malice, a Malignant Tunnel just to the South of Nostrava, aka southwest of Fractured Peaks. The Fissure has 3 Elites, a end boss, and a optional Outgrowth, which doe spawn Wrathful Outgrowths, as you can see from the image at the top of the page. However, be aware grabbing Wrathful Invokers are very weighted, and so, you may need to gamble a lot before you get one of these. It’s something to be mindful of.
The only other option you have is to fight the Echo of Varshan. This creature drops a random Caged Heart of any type, meaning you could get a 1/32 chance in theory to get what you want. Add in the better Malignant super drops, and your odds increase ever so slightly of getting The Barber, among other Super World Tier items. But, its the least resource efficient method as you need a Foul Invoker, or a Tormented Invoker to do that. Not to mention the fight can be difficult, as it’s typically level 60 or 80, depending on World Tier.
