Wrathful Hearts in Diablo 4 are the lucrative Malignant Heart type, which players will want to farm when they begin to enter the World Tier 3 and 4 progression tiers. The reason for this is because they contain some rather powerful effect, typically bolstering a large variety of playstyles, so a build always has something to really ramp their styles up. So, if you’re interested in unlocking you final form in Season 1, here’s how to get Wrathful Hearts in Diablo 4.
How to Get Wrathful Hearts in Diablo 4¶
There’s a few methods to get Wrathful Hearts in Diablo 4. However, both of them are somewhat luck based. There’s another method too, yet, they are even more luck dependent than the other two options on the list. These methods are:
- Uncertain Hearts
- Wrathful Invokers
- Random Drops from the Echo of Varshan
Uncertain Hearts¶
The two main methods you are likely going to encounter Wrathful Hearts in Diablo 4 are via the Invokers or random heart chances. Uncertain Hearts are your gamble of Season 1, somewhat acting like the Obol gambling from the Purveyor of Curiosities. You can spend 20 of each type of Ichor (Brutal, Devious, Vicious), and it will grant you a random Heart form the full list of 32 Malignant Hearts in Diablo 4. However, they are weighted, as some of the Malignant Hearts are super rare, and only drop under certain World Tier setting, typically in World Tier 3, then a few are World Tier 4 only. So, the chances are actually a lot lower. We recommend saving these rolls for one you beat the Cathedral of Light Capstone Dungeon and ascend to WT3 at the earliest. That way, you can find some of the WT3 options in the pool, and adding an extra Wrathful to the available pool shrinks the odds away from the other hearts somewhat.
Wrathful Invokers¶
As for the second option, players can gamble their luck on creating a random Invoker. If you take the Uncertain Invoker option at the Research bench, it will cost 40 of each Ichor type, and you will receive a random Invoker. If you get lucky and acquire a Wrathful Invoker, then you may use it in Malignant Tunnels. From then on, you can walk into a Malignant Tunnel, look for the optional room with the Malignant Outgrowths, and find the Wrathful Outgrowth. Keep in mid that each tunnel can either have a room with a Monstrous Outgrowth in it, allowing you to spawn the seasonal boss. Or, it will contain two random heart types, meaning it is possible to complete a Tunnel, and not get a Wrathful Outgrowth. If you do get a Wrathful, it allows you to use the Wrathful Malignant Invoker on that Outgrowth, and then spawn a Malignant Foe. Once its defeated, you may acquire a cage heart, which will give you a random Wrathful Heart. It will either be your class’s wrathful, or one of the Wrathfuls all classes can use, presuming it drops on your world tier.
Echo of Varshan¶
The final method is the Echo of Varshan boss fight. As alluded to in the paragraph above, you may come into some rooms at the end of the Tunnels that contain Monstrous Outgrowths. These Monstrous Outgrowth require either an Invoker of Varshan (World Tier 1/2), or a Foul Invoker of Varshan for World Tier 3 and the Tormented Invoker for World Tier 4 and the Uber Varshan fight. If you consume one, you have a boss to beat. If you beat it, you can get lots of loot, some Ichors, and a Caged Heart. The Caged Heart can be any Heart of any type, just as long as your class can use it, or it can drop on your World Tier. So, again, the odds a favored slightly more to the Brutal, Devious and Vicious, but, it can be a decent way of grabbing Wrathfuls too.
Also, it seems the two Malignant Tunnels that host the Echo of Varshan fight are as follows:
- The Boiling Wounds - Eastern Fractured Peaks
- The Ravening Pit - Untamed Scarps of the Dry Steppes, near The Ruins of Qara-Yisu.
There’s likely more, but these two are the ones we know for sure have the boss room present.

So, if you were to use a Wrathful Invoker on World Tier 2, you will only get the chance to get Creeping Death or the Malignant Pack. If you use it on WT3, then The Barber gets added to the drop pool. And, on WT4, you class specific opens up to you. Lets take The Barber as an example. If you’re running a crit build, this could be a great choice. So, you ideally want to farm it on WT3 for better chances of getting that item. If you want you class, you can only attempt it on WT4.
